2. Form may be filled in on computer then printed and faxed or mailed.
3. Check Software version for the checks.
4. Select the quantity, color, starting number  and other options.
5. Attach a voided check from your existing supply or your bank's MICR specification sheet. Note any changes on the face of the check.
6. Attach your check, provide account draft information or send credit card number.
7. Shipping Address: (Please Provide Name & Street Address)
